maximum wellness advertisements

While brainstorming for ideas for my ad I thought of showing people three different activities maximum wellness will help people accomplish. The activities might seem easy to most people but when their backs in pain, their unable to accomplish different things in life. So I'm thinking of using either typography or images to demonstrate the concept.

Ideas....for my mailer

So I'm thinking about creating a mailer for Maximum Wellness that will inform patients of their monthly chiropractic visits as well as extended information about chiropractics.
I want patients to keep the mailer so I want to incorporate a perferated calendar.
I thought about making the mailer into a shape of a butterfly, so when patients open it up they'll be suprised to see a butterfly. I also thought of making the mailer a pamphlet but I dont want to waste acess paper. Right now im contemplating the shape of my mailer. I want it to be clever and unique but still functional in the mail.
